Adding Form Scoring Categories

After a form are created and scoring is added, you can create Form Scoring Categories. You can define names for the scoring ranges, e.g. Low Risk 0-5, Mod Risk 6-10, High Risk 11-15. You can also set up form schedules (e.g. if a resident scores as high risk then a follow up form is scheduled q2wks), and triggers (e.g. if a resident scores as high risk then Focus/Goals/Interventions trigger to the Service Plan and/or Alerts trigger to the Care Services Dashboard).

Some of the System forms have scoring categories already assigned, such as Braden.

Note

If you create scoring categories based on risk, you need evidence-based documentation to back up the scores.

Note

A Category Description cannot be changed after saving, but you can click del to remove the category and add it again.

  1. Do one of the following:

    • Single community: Care Services > Setup > Form Scoring Categories.

    • Multi-community: Management Console > Standards > Care Management > Form Scoring Categories.

  2. Standard Forms - select the form.

  3. Available Scores - select the score.

    1. Category Description - define the name of the scoring range, e.g. Care Level 1.

    2. Low – enter lowest score threshold, e.g. 0.

    3. High – enter highest score threshold, e.g. 5

    4. Sequence – enter the order you want the category to appear, e.g. 1.

  4. Repeat the above steps to create additional categories, e.g. Care Level 2 (Low: 6, High: 10, Sequence: 2) and Care Level 3 (Low: 11, High: 15, Sequence: 3).

  • The following options are available for form scoring categories, however they can also be set up based on a specific score, as opposed to a score range.

    Adding Scoring to A Form

    • Schedule - click schedule for the category to set up a formschedule based on a form score range. Click New > enter Description, select Triggering Type of Score, enter Score Value, select Trigger Form, select schedule > click Save.

    • Trigger - click trigger for the category to set up a trigger based on a form score range. Enter a score value > click New for Care Plan Triggers or High Risk Alert Triggers > select item(s) > Save.
