Completing Risk Forms in the Skin & Wound Mobile App

You can complete risk Forms in the PointClickCare Skin & Wound mobile app that are specific to skin integrity and wounds. Completing risk forms from the mobile app increases efficiency in documentation.

When a form is scheduled from the Forms tab of the resident's chart, you see a red dot on the tile in the mobile app to indicate a form needs completion.

After creating a form, the form tile appears on the resident's home screen in the mobile app and in the Forms and Skin & Wound tabs of the resident's chart in the web application.

Login to the PointClickCare Skin & Wound mobile app.
Click the resident icon or swipe to the right to see the resident list.
Select resident.
Do one of the following:
- To complete a scheduled risk form - Click the tile for the scheduled form.
- To create a new risk form - Click + to add a form, then select type of form.
Click the questions or items and select response(s).
Click Save.
To lock the assessment, click the tile for the assessment.
Click the padlock icon and select Sign + Lock or Sign to complete the form.

When completing the PCC Skin & Wound - Total Body Skin Assessment in the mobile app, if new wounds are indicated, a placeholder icon shows on the resident's home screen. You must click the placeholder icon to create the wound form to avoid duplication.
A form cannot be unlocked if it is configured to create a Structured Progress Note. A message shows if you try to unlock a form with an SPN from the mobile app.

An icon appears on the resident list when a form has not synced. Unsaved images and assessments are permanently deleted if you continue to log out before syncing. After syncing, the icon no longer appears.
