Form Schedule Audit Report

The Form Schedule Audit Report is found under the Care Services Reports menu under the Forms heading. The Form Schedule Audit Report allows you to view when a form was scheduled, when it was due and when it was actually completed, and also indicates if it was completed on time, ‘X’ days early or ‘X’ days late. This report assists in auditing schedule information for forms across your community.

You can select the following options/filters:

  • Resident Number/Name.

  • Unit/Floor/Status.

  • Sort By: Resident Name, Date Form Scheduled, Due Date, Form Completion Date.

  • Date Range and is based upon Due Date.

  • Form (required).

The report output contains the following columns:

  • Form – name of form scheduled.

  • Scheduled Date – based on manual or activated schedules for the form.

  • Triggered By – schedule event type that triggered the form schedule.

  • Due Date – based on the schedule, when the form is due.

  • Created Date/By – user and date form was created.

  • Completed/ Date/By – user and date form was completed.

  • Completion/Due Date Comparison – compares due date to when it was completed and marked as ‘on time’, ‘x day(s) early’, or ‘x day(s) late’.

  1. From the Form Schedule Audit Report on the reports listing, select your Report Options.

  2. Select your Filter Options.

  • The report can be run for a single resident for up to 10 forms or for all residents for a single form.

  • Schedules that are deactivated or cleared are not included in the report.
