Service Plan Report

The Care/Service Plan Report generates a printable view of each resident's current Service Plan and/or all Service Plans (for a single resident only). You can include Tasks, Revision History, Resolved/Cancelled items, or department specific Service Plan information. You can also select to show Created, Revision and Target information for Focus, Goals and Interventions as selected.

  1. Reports menu > Service Plan.

  • Can also run a Blank Copy of a Service Plan.

  • The Sort By: Resident Name, Location or Department with a secondary sort available for Revision Date; when this sort option is selected and the report is run, the content is sorted based on the Date the intervention was revised and Intervention Date Initiated.

  • Use the Most Recent option to run multiple resident Service Plans in your community. This reports on all resident's Service Plan as they stand today. If a Service Plan is closed, it pulls the Service Plan as it was most recently.

  • Use All to run any/all Service Plans for a single resident. You cannot select All until you enter a resident Name/Number.

  • The option to filter by Department only reports on an individual Department's Service Plan items. Otherwise runs for All Departments.

  • You can select to Insert Page Break After Every Focus check box.

  • When printing a Service Plan from a resident's chart, the order of items is based on the Sort By filter. For example, if Sort By is set to Library Order, the Service Plan prints in the library order (alphabetical).

  • Care Plan Type must be configured to show on this report.
