Copying A Form

You can copy a form to use as a template for a new form. Copying a form creates a new, independent copy of the original form, under a new form name. Changes to the new form do not have any effect on the original form, resident forms created against it, or schedules.

  1. Click copy for the form>click OK.

  2. Enter a unique Description and make other changes if required in the New Standard Forms page > Save. The new form appears in the Under Construction section.

  3. Click edit for the new form and make changes as needed.

  4. Click make available for scoring for the new form and add/amend scoring/Structured Progress Note.

  5. Click make available on-line for the new form.
