Overview of Structured Progress Notes

A Structured Progress Note (SPN) can be added to a form in the Standard Forms Library. An SPN allows you to build the content of a Progress Note that pushes to the Prog Note tab of a resident's chart upon completion of the form. Content for an SPN can populate from multiple areas, including responses, narrative responses, resident profile, scores, and score categories.

If you want to add narrative responses that can be used in the construction of the SPN, this must be done while the form is in the Under Construction section. They cannot be added after the form is moved to Ready for Scoring/SPN Setup and the SPN is created.

Creating Narrative Responses

After the form is built and the narratives are added (if applicable), the form must be moved to the Ready for Scoring/SPN Setup section to create the Structured Progress Note.

You must create a new Progress Note Type to use in the Structured Progress Note.
