Adding Question Groups

After creating a form and adding sections (if applicable), you can add question groups to group similar questions together. You can have one question group or multiple question groups per form or form section.

Note

After a Question Group Number/Letter is assigned, it cannot be changed.

  1. Click edit for the form.

    • If in a multi-section form, click edit for the section you want to add the question group to.

  2. Click Create New Question Groups where you want to add a new group.

  3. Click Choose to select the Question Group Layout.

    • Vertical - The Vertical format places the answer area directly under the question being asked. This format is often the most readable, but makes the forms longer.

    • Side-by-Side - The side-by-side layout places the answer area next to the question. This format may be preferable for lists of check boxes or question groups where many of the questions use the same control.

    • Two-Column - The two column layout allows you to have questions set up on both the left and the right side of the screen. This format may be preferable for lists of check boxes or question groups where many of the questions use the same control.

  4. Enter a unique Question Group Number/Letter, a Group Title and any optional Group Text.

  5. Add question(s).

  • Click edit for the question group title to amend the Group Title, Group Text, or Layout Style.
