Creating Forms

You can create a new form to evaluate a resident. If the form is scheduled, you can click + to create the form from the scheduler. If the form is not scheduled, you can click New to create the form.

Warning

When creating a form from the scheduler, it is recommended to keep Check all schedules to be cleared/updated upon completion of this assessment selected, otherwise the form remains on the scheduler after it is completed.

Note

A form is generally created in a resident's Care Services chart, but can also be created in the Billing chart, Marketing, or CRM, if configured.

    • Click + for Next Form Due > click on a form name.

  • You can also create a new form by clicking copy for a completed form of the same type. The responses autopopulate from the previous form, and must be reviewed for accuracy and acknowledged or modified to reflect the resident's current status.

  • If configured, you can create a new form in Marketing or CRM.

    • IRM: Resident's Inquiry Detail page > Assmnts/Verifications > click New.

    • CRM: Resident's Lead Details page > Forms > click New.
