Creating a New Form

You can create a new form in the Standard Forms Library. There are several configurable options.

Description - must be unique.

Schedules - you can add a schedule to a form which can assist your staff in knowing when a particular form is scheduled to be completed.

Multi-Section - you can add sections to a form and each section can be completed/signed by different staff members, if required. Note - if selected and saved, it cannot be cleared.

This form can be created from - designates which modules this form can be created from: Billing, Care Services and/or Marketing.

If created and completed in the Marketing Module: This completed form can trigger to and is visible from Care Services Historical Form View? - if selected, then after creating the form in the Marketing Forms/Verifications tab, the form flows to the Care Services module when the resident is admitted into the community . Users must click the historical records icon to view the form. The form appears in the Forms tab in the Care Services resident chart in the Assmnts Hx section.

Scope - select the organization scope for this form.

Include in Header:1st Page and Subsequent Pages - select to indicate which heading fields to include on the form.

Header Logo - click the magnifying glass to select an image for the Header Logo.

Include Signature Section in Report - select this to add a signature section to the printable version of the form.

By Default Print the Short Version of the Form Report - select this to print only the responses that are selected instead of all response options.

By Default Include Disabled Questions in Report - select this to print disabled questions. When this option is not selected, these questions are not included on the report and reduce the size of the report.

Show on Shift Report and Show on 24 Hour Report - select to show the form date and title on the Shift report and 24 Hour report.

Include in Daily Summary Metrics - select this to show this form in the Daily Summary dashboard pane.

  • You can edit the configuration options for your form by clicking on the form name in the Custom Form Library.

  • You can also create a new form by copying an existing form.
