Monitoring and Managing Service Plan Reviews

After a resident service plan is completed, it is recommended the service plan be reviewed quarterly, or periodically based on your community requirements. The service plan review provides a way to perform and document periodic review of the resident's interdisciplinary service plan.

Note

After a review is created, you must make any edits to the Service Plan through the Service Plan Review History section.

  1. Do one of the following:

    • Care Services > Dashboard > Service Plan Reviews Overdue/Due > Find Resident with upcoming Review Not Started.

    • Care Services > Resident > Service Plan.

  2. Click New Review.

    • Complete the fields as needed and click Save.

  3. Click edit in the Review History section of the resident Service Plan

    1. Click edit next to the department.

    2. Click View Triggered Items Now to review any suggested service plan items.

      For more information on managing triggered items, see Managing Triggered Items in Service Plans.

    3. Click edit for the Focus(es), Goals(s), and Interventions(s) to be updated. Make changes as needed, and click Back to return to the Service Plan Reviews page.

  4. Click sign after editing the department review.

  5. Click Complete Review when all departments have signed the Service Plan review.

    Note

    Communities can designate staff to ensure the Service Plan review is signed off in a timely manner. Follow your community policies.

  • The Service Plan review can be printed for the resident or family to sign by clicking print next to any review.

  • After the Service Plan Review is complete, it is important for a designated staff member to remove any remaining triggered items.
