Managing the Standard Forms Library - Sections

The Custom Form Library consists of four sections. Each section contains links, the form description, and the form status.

  • Under Construction - Contains forms that are being created. After all structural changes (sections - if applicable, question groups, questions, and narratives - if applicable) are complete, click make available for scoring to move the form down to the next section.

  • Ready for Scoring/SPN Setup - Contains forms that are structurally complete. After scores and/or Structured Progress Notes are added, click make available on-line to move the form down to the next section.

    Note

    A Form is immediately available for selection in a resident's chart after clicking make available on-line.

  • On Line - Contains forms that are in use (in the communities selected in the communities link). Online forms are available for selection in the Forms tab of a resident's chart. If a form is no longer needed, click retire to move the form down to the next section.

  • Retired - Contains forms that are no longer in use. Retired forms can be copied to create a new form, or moved online again.
