Associating Form Scoring Categories

The form can trigger scores associated to Service Levels Type and Points Type Service Package Categories. To automatically trigger Service Notifications, the Care Services staff create form scoring ranges and scoring categories. You need to complete Billing Setup by associating the form Score(s) to either Levels or Points Type Billing.

In form Scoring Categories, select whether the form score triggers Service Levels or Points type charges in Service Packages. The score determines the number of units in the resident's Service Package. For Service Level billing, associate the Score Ranges to the Service Levels. For Points billing, associate the Score to the Points Service Category, and define the number of score points equal to one unit of the charge code.

When the form is completed, the score determines the Service Level and/or Points charges in the resident's service package. If there is no Service Package, or the service level is different, and no approval process is required, the service package is created. Otherwise, a Service Change Notification is created for approval.

Warning

To calculate accurate Points-based charges, the unit of measure used to complete the form, such as, per minute, per hour, or per visit must be the same as the unit of measure used to setup Form Scoring Categories. It is important Care Services Staff documenting the form and Billing staff completing this setup understand the units of measure used in both the Care Services and the Billing modules.

For example, the charge for Bathing is $30 an hour. The default charge code amount is $30. The unit of measure is minutes. 60 points is 60 minutes. The points billing is set up to charge per minute. # Points is set to 60 = 1 unit of charge code. To calculate the correct charge for time spent bathing, in the form, you must enter bathing time in minutes.

  1. Do one of the following:

    • Single community: Billing > Setup > Form Scoring Categories

    • Multi-community: Management Console > Standards > Financial Management > Form Scoring Categories

    Do the following: 

    1. Select the form. Scores for the form appear below.

    2. Next to each Available Score, select the Service Package Category type.

  2. For each score set to bill Service Levels, associate a Score Category (range) with the Service Package Category and appropriate Service Level.

    1. Click the Service Level tab.

    2. Select the form from the list. Available Score Categories appear below.

    3. Select an Available Score.

    4. Next to the Category Description, click edit.

    5. Select the Service Package Category

      Note

      To receive Notifications when you do not bill for a score range, such as a low score range: 0.00 - 2.00, select None in Service Level. For residents with an existing Levels Service Package, if you lock the form and the score is associated to Service Level, None, a notification generates.

    6. Select the Service Level. If a Service Package is not billed for a score range (for example, 0.00 - 1.00), select None

  3. For each Available Score set to Points Billing, associate the Service Package Category and define the number of Points equal to one unit of the Charge Code.

    1. Click the Points tab.

    2. Next to the Available Score, click edit.

    3. Select the Service Package Category.

    4. Enter the # Points equal to one unit of the charge code.

  • You can create Form Scoring Categories in Care Services. You can view the scores ranges by clicking the button at the top of the Form Scoring Categories page: Care Services at the community and Clinical at EMC. When you associate Form Scores to Service Package Categories and Levels, the information helps you determine the appropriate Service Level for each score. To use this view, you must have at least read Security access to the Setup-Assessments option in the Clinical Module.

  • In a multi-community database, at the community level, Standard Form, shows only the Form activated in that community. At Management Console, Standard Form shows all forms in the organization.
