Change Log Report

The Clinical Chart Change Log Report can be generated to view any change made to a Clinical Chart template(s). This report shows a history of changes to elements or settings in the template(s) and can be run at both the community and the Management Console for multi-community organizations.

The report shows the following based on selected report options:

  • Clinical Chart Name, Scope, Created Date/Time, and Created By.

  • Revision Date/Time, Revision By, Module/Report Name, Action, Field Name changed, Original Value, Updated Value.

  • Care Services > Reports > Clinical Chart Change Log Report.

  • Management Console > Reporting > Care Services Reports > Clinical Chart Change Log Report.

  1. Do one of the following:

    • Care Services > Reports > Clinical Chart Change Log Report.

    • Management Console > Reporting > Care Services Reports > Clinical Chart Change Log Report.

  2. Select Date Range.

  3. Select User or leave blank for all users.

  4. Select Clinical Chart Template(s) listed.

  5. Select to Group By.

  6. Select Sort By (Management Console only).

  7. Select Report Output Format.

  • Click Display Filters to filter the chart template listing by Clinical Chart Name, Status and Scope (Management Console only) and click Refresh

  • Select All Clinical Charts and sort the chart templates by Name, Scope or Status by clicking on the column heading.

  • You can run this report as PDF, CSV or Excel format.

  • Only Clinical Chart templates with changes appear on this report.

  • Community level report does not allow for Group By and Sort By. At Management Console level, if you select Group By=Chart, Sort By=Date. If you select Group By=Scope, Sort By=Chart.

  • Clinical Chart must be enabled in your community.
