Managing Triggered Items in Service Plans

Triggered items are suggested items to add to your resident service plan. Depending on your configuration, it is possible for certain Focus, Goals, or Interventions to trigger to your service plan, based on answers to form questions and progress notes. Triggered items should be reviewed to determine if they are needed within the resident service plan by members of the care team. Missing items not saved to the service plan should be removed from the triggered items area.

  1. Care Services > Resident > Service Plan.  

  2. Click View Triggered Items Now.

  3. Select Focus(es), Goal(s), and Intervention(s) to include in the resident Service Plan.

  4. Click Edit for each Focus(es), Goal(s), and Intervention(s) requiring further personalization.

  5. To remove all triggered items, select View Triggered Items Now, and click Remove All.

    Note

    Communities should have a process in place for someone to remove all remaining triggered items after the Interdisciplinary Team has completed their service plan review. Triggers are for the whole team , and on a regular basis, when completing the service plan, clean up of the triggered items is necessary.

  • There is an abbreviation next to any focus, goal, or intervention triggering from progress notes (PN),  a form (UDA), and MDS. Hover over the abbreviation to see the date and source of the triggering item. If enabled, you can also see PATH if items are triggered from a care pathway.

  • You can regenerate triggered items by clicking the Regenerate Triggers button within an appropriate form or MDS.
