Form Scoring Report

The Assessment Scoring report allows you to access any type of scoring on an assessment that is completed using most recent client assessment within the desired date range. This report can be configured for a specific type of assessment within a specified time frame, and filtered by unit, floor, status, and reason for assessment. Available scores to report on are based on the Assessment Type selected from the list.

Procedure

Billing > Reports.

  1. Resident Number - leave blank to include all residents. For a specific resident, enter the resident number or click the lookup.gif to assist you.

  2. Resident - leave blank to include all resident. For a specific resident, enter the resident name or click the lookup.gif to assist you.

  3. Unit - Select the list for a specific unit.

  4. Floor - Select the list for a specific floor.

  5. Current Residents Only - Select this option if you want the report to show current residents only.

  6. Date Range - You can manually enter a date in the fields or click the calendar.gif to select a date.

  7. Report on most recent assessments only - Select this option if you want the report to show the most recent assessments only.

  8. Exclude assessment when Outcome Measure could not be calculated for all selected scores - Select this option if you want the report to exclude assessments when Outcome Measure cannot not be calculated for all selected scores.

  9. Assessment Type - select assessment type from the list.

  10. Scoring - click the lookup.gif for scoring options.

  11. Assessment Status - Select the list for specific assessment status options.

  12. Secondary reason for assessment (AA8b) -  Select the list for other options.

  13. Payer - You can click Clear All to clear all payers, and then select Specific Payers.

  14. Run Report.

Hints and Tips

  • An Assessment Type must be selected, and any/all scoring associated with that assessment can be selected within a date range.
