MDS 2.0 - Section V - RAPs Worksheet - Combine RAPs and Use Standard Statement

If your facility uses the New RAP Worksheet format for working RAPs, you can combine RAPS and use the Standard Statement (if enabled) in the RAI-MDS 2.0 assessment. Both of these features allow for gained efficiencies while completing RAPs information.

Combine RAPS

  1. From the resident chart, click the MDS tab.

  2. Click Edit next to the desired MDS assessment.

  3. Edit Section V.

  4. Edit the RAP you want to work.

  5. Next to the 'Describe impact of this problem/need on the resident and your rationale for care plan decision' section, click Combine RAP link.

  6. Click choose link to combine the impact statement documentation for two RAPs which are closely related.

Standard Statement

  1. From the resident chart, click the MDS tab.

  2. Click Edit next to the desired MDS assessment.

  3. Edit Section V.

  4. Edit the RAP you want to work.

  5. Next to the Describe impact of this problem/need on the resident and your rationale for care plan decision section, you may see a Use Standard Statement link. This link option only appears if the RAP triggered is an existing RAP(=) for a Quarterly assessment type. This option allows you to use the following standardized statement that should be edited further for the resident: This is an existing RAP. Resident is responding to the interventions as outlined in the care plan. His/her clinical assessment has not changed from the last assessment. Care plan goals and interventions are reviewed by the care team members and continue to be effective in (preventing, improving or maintaining) the RAP problem.

  • The New RAPS worksheet is enabled with the appropriate security permissions through Clinical > Setup > MDS Configuration.

  • The Combine RAP option only appears if New RAPs Worksheets are being used and 'Allow users to combine RAPS' is set to Yes in MDS Configuration.

  • The Use Standard Statement option only appears if New RAPs Worksheets are being used, 'Use Existing RAP Standard Statement' is set to Yes in MDS Configuration, and the triggered RAP is an existing RAP(=) for a Quarterly assessment type.

  • You must have appropriate security permissions for section V of the MDS to work RAPs.
