Integrated Assessment Record (IAR) – Entering Consent – MDS Tab

(Skilled Nursing Ontario Facilities only)

Some Ontario facilities use the Integrated Assessment Record (IAR). If your facility is participating in IAR, you can enter a resident’s IAR consent through IAR Consent in the resident’s MDS tab. After you enter IAR Consent, the consent autopopulates MDS assessments on a go forward basis until the Consent Expiry Date is reached.

IAR consent can also be entered in each individual MDS assessment. For more information, see Integrated Assessment Record (IAR) – Entering Consent – MDS Assessment.

  1. Navigate to Clinical > Resident's Chart > MDS tab.

  2. Click + next to Next Full to expand the section.

  3. Click IAR Consent.

  • After consent is entered, IAR Consent updates with Granted or Deny.

  • Consent autopopulates assessments that are opened after consent is granted or denied in the MDS tab. It does not autopopulate existing assessments.

  • IAR Consent can be overwritten within an MDS assessment if it differs from the consent selected in the MDS tab. Click the Edit link. Correct the fields and Save. The IAR Consent section of the MDS tab does not update with the changes made in the MDS assessment.

  • After the Consent Expiry Date, IAR Consent shows "Pending", and the expired date appears. Consent must be entered within the MDS assessments until the consent is updated in the MDS tab.

  • The IAR Consent must be entered to lock the MDS assessment.

  • When "Do you wish to collect the IAR Consent Date?" is set to Yes in MDS Configuration, the Consent Date field is mandatory, and populates with the current date.
