Documentation of Restorative Interventions from the Care Plan in POC

Restorative Care Plan Interventions are located in CAA-05 of the PointClickCare Default Library for Focus Categories ADL Self-Care Performance Deficit and Limited Physical Mobility. These interventions are configured to show either on a Paper Care Record (in a non-POC enabled database) or in POC when the intervention is added to a resident’s care plan. The answers of follow-up questions are attached to the interventions. You can find them in Section O of the MDS for Restorative minutes (Amount of Minutes spent training and skill practice in XX).

  1. From the resident Care Plan, select the appropriate Focus from the CAA-05 category and add to resident care plan. Specify Etiology and Goal.

  2. Select the appropriate Nursing Rehab/Restorative Intervention.

  3. Edit and personalize Intervention as needed including description, position, frequency.

  4. Save. The Intervention appears in POC for documentation.

  • All Restorative Interventions are called Nursing Rehab/Restorative. When you enable POC in a facility, a pencil icon appears next to the intervention.

  • System totals number of minutes. You can find this total in the MDS Section O in 15 minute increments when answered in POC.
