Managing MDS Triggers, Warnings, and Look Backs

Triggers, warnings and look backs are helpful to streamline the workflow when completing the MDS assessment.

You can configure MDS questions to trigger care plan items, tasks, assessments, and alerts upon completion of an MDS assessment. If your organization is using the default Care Plan libraries, there are triggers already created. 

You can create warnings to trigger when questions are coded a certain way.

As part of POC configuration you can create links for POC documentation to show in the tools or look back icon of the MDS.  If your organization is using the default Follow Up Questions, there are look backs already linked to specific MDS items. 

Triggers 
  1. Do one of the following: 

    • Single facilityClinical > Setup > MDS Triggers.

    • Multi-facility: Management Console > Standards > Care Management > MDS Care Plan Triggers.

  2. Select section and question.

  3. Click triggers.

  4. Select Triggers for Response.

Warnings
  1. Do one of the following: 

    • Single facilityClinical > Setup > MDS Warnings.

    • Multi-facility: Management Console > Standards > Care Management > MDS Warnings.

    Note

    For multi-facility, you must also go to the facility level setup > MDS Warnings and manage the System Warnings section. 

  2. Select MDS section and question.

  3. Click warnings.

Look Back

POC must be enabled to use look backs.

With POC setup, you can also configure your Look Back Report setup. See Managing Look Back Report Setup

  1. Do one of the following:

    • Single facility: Clinical > Setup > MDS Triggers.

    • Multi-facility: Management Console > Standards > Care Management > MDS Care Plan Triggers.

  2. Select MDS section and question.

  3. Click lookback.

  4. Make selections and complete the fields as required.
