Skilled Nursing Facilities

Creating Simple Alerts

Simple alerts can be triggered by the occurrence of a single documentation response. For example, your facility can set an alert to trigger every time a POC user documents that a resident refused their meal.

This type of alerting can sometimes be less effective because the system is alerting the nursing staff to every occurrence of an event and it does not necessarily highlight situations where there is truly a problem. For example, a resident may refuse their meal because their family brought them a snack right before dinner. In this case the resident likely isn't experiencing a change in condition.

Simple Alerts may also be hot listed to allow users to manually trigger alerts from POC or from the Clinical Dashboard.

  1. Views: click Simple Alerts.

  2. Enter Description. For example. "Resident did not eat meal".

  3. Select Module - for alert to appear in Clinical and/or Admin.

  4. Enter Order number if needed.

  5. Select Alert Hot List to allow this alert to be manually triggered by staff.

  6. Change the scoping if applicable.

Note

To associate a simple alert with a follow up question, go to EMC > Standards > Care Management > Follow Up Question Library > click the alert link to the left of the appropriate fuq > simple alerts section: select trigger response from list > click new > select simple alert > save.

  • Keep hot listed alerts simple and to the point.

  • You can also create Custom Alerts from POC and the Clinical Dashboards.

  • Consider creating a Simple Alert that can be manually triggered: “Changes to Care Plan/Kardex – Please Review” to alert users to look at the resident Kardex or Care Plan Report from within POC. This alert can be triggered from within the resident’s care plan detail page to alert caregivers of changes to the care plan/kardex.
