MDS 2.0 - Symbols Legend

When entering in responses into the MDS assessment records, you see various different symbols found next to each MDS question. These symbols provide additional information in the MDS that may be useful to you when completing the assessment record.

  • RUG - Indicates that the question is used in calculating the RUG Score.

  • RAP/CAP - Allows you to view the related RAPs/CAPs triggers when you hover your mouse over the symbol.

  • Q - Indicates that the question can impact the QI/QMs. When you hover your mouse over this symbol, you see items that are related to that question.

  • H - Allows you to view all past responses for, or the history of, this question. Click the H link to access this information.

  • E - Indicates a response was edited during the completion of this assessment.

  • TRIG - Allows you to view the triggers that are sent to the Care Plan, User Defined Assessments (UDAs) that are scheduled, and alerts that might be generated based on the response to that question. Click the TRIG link to access this information.

  • ? - Allows you to identify where the documentation is coming from for fields that are auto-populated from other areas of the system (POC, Therapy Module, UDA). Click this icon to access the information.

  • note - Appears next to each MDS question, and allows you to add MDS Notes to support or describe the rationale for coding decisions. When clicking on the note link, you can create a New note or select an existing note already attached to an MDS item in the current assessment. These notes are visible within the corresponding RAPs, which aid in the accurate completion of care plan decision making.

  • When a note is added, the link changes from blue to red.

  • ? shows information from POC, Therapy Minutes, and UDAs if your facility is using these modules and appropriate setup is completed.

  • The Note feature must be enabled in MDS Configuration to see these links in the MDS assessment.

  • You see RAP or CAP depending on how your facility is set in MDS Configuration.
