Titration Schedule

Certain medication orders require titrating doses over many days or weeks. Titration Schedules from within orders allows you to enter varying doses over periods of time all in a single order. Titration orders can only be applied to the Order Category of Pharmacy because only medications with doses can be titrated up or down. Titration orders have varying End Dates applied, depending on the order written.

Specific Date

  • Continue the order until a specific date and time.

Duration

  • Allows for the order to be scheduled for a specific day(s), week(s), or month(s), and shows the end date based on the duration entered.

  • Select to administer until the schedule is finished.

  • Select to require a reassessment. This prompts an alert to reassess the order after schedule completion.

  1. From a new order, access the Scheduling Details section.

  2. Select Titration to add a first titration schedule. Titration 1 appears on a tab for entry of a titration schedule.

  3. Enter the Dose or Admin Quantity if prompted.

  4. Enter Frequency that the order needs to be administered.

  5. Enter the Schedule Type for the routine order.

  6. Enter the Facility Time Code that applies to the Frequency selected.

  7. Review Start Date, and Time. This field shows the next date/time from the Order Date/Time based on the Frequency, Schedule Type, and Facility Time Code entered.

  8. Select an End Date.

  9. Enter any Admin Notes or Supplementary Documentation via Add.

  10. Enter in the Related Diagnosis and/or the For (Indications for Use) fields.

  11. Enter Additional Directions as directed in the order.

  12. Select the Administered By.

  13. Click Next Titration to add another titration to the order.

  14. Continue on to the Source Details section of the order.

  • After the first Titration schedule is entered, Titration 2 and beyond allows for a maintenance dose to be entered with an End Date of Indefinite if that is required based on the order prescribed.

  • Either the Related Diagnosis or For (Indications for Use) field are required for orders entered in with a Titration schedule. If neither are entered, an error message appears after saving or queuing the order.

  • Admin Notes are notes that are visible to the clinician administering the order to the resident.

  • Supplementary Documentation can be added to any Routine schedule for a variety of system items or a Custom item can be created for documentation to be recorded against when the clinician is administering the order.
