MDS 2.0 - Section V - RAPs Overview

Resident Assessment Protocols (RAPs) are used in some Canadian provinces to analyze, make referrals (if configured), and assist in clinical decision making about a resident issue or problem. RAPs assist in identifying areas of an actual problem area or potential issue. Up to 18 RAPs can potentially be triggered from the RAI-MDS 2.0 assessment. RAPs are located in Section V of the MDS assessment.

MDS assessment Types that contain RAPs: 01. Admission Full Assessment, 02. Annual Assessment, 03. Significant Change in Status Assessment. Depending on your province, you may also see RAPS in Quarterly assessments.

  • A RAP shown in green indicates that it is triggered.

  • A RAP showing (=) indicates that the RAP is triggered in the previous MDS by the same response(s).

  • A RAP showing (Δ) indicates that the RAP is triggered in the previous MDS by a different response(s).

  • A RAP showing (+) indicates that the RAP is newly triggered, and was not triggered in the previous MDS.

  • Click the H (History) link next to a RAP to open the history of the RAP in the current and past assessments.
