Requiring Documentation of Removal

Certain pharmacy orders require a clinician to document the removal of orders before the next administration of that order. For example, you want to document the removal of a patch before administering the next patch onto a resident.

Depending on the medication selected, the Document Removal option allows you to force documentation of the removal of that order at a given time.

Warning

When adding an end date to an order that requires documentation of removal, the end date only applies to the application. The documentation of removal is based on when that application was last completed. For example, if a 3 day patch is applied on Monday, the final removal documentation is due Thursday, regardless of the end date. 

  1. From within orders where application of a medication is necessary (ex. Fentanyl patch), complete Required Fields up to Facility Time Code.

  2. Select Document Removal.

  3. Continue completing the remainder of Schedule Details section until you complete review of the Apply Pass Time.

  4. Enter Remove Pass Time or select to Remove After X hours.

  5. Continue with remainder of Order Entry.

  • Only certain medications selected allows for Document Removal to appear in Routine and PRN Schedules.

  • Use the Pass Time to document removal if you required the order to be removed at a certain time or just before the next administration.

  • Use the After X Hours fields if you want the order to be removed a certain number of hours after administration.
