Understanding Frequencies in Point of Care (POC) Quick Reference Guide

This quick reference guide provides an overview of frequencies used in POC.

Specific Time

The task must be documented at a specific time. For example, a meal task is documented at 0900, 1300, and 1800, or a safety check task is documented every hour.

Timed tasks can be documented 1 time, and can only be documented when the task tile turns yellow, indicating documentation is due now.

QShift

The task must be documented at least 1 time per shift. For example, a dressing task is documented 1 time, or a Behavior task is documented 2 times during the shift.

QShift tasks can be documented multiple times.

PRN

The task is documented as needed. For example, a transfer task is documented when the resident requires assistance.

Monthly/Every X Months

The task must be documented 1 time per month or every X months, either at a specific time or on a specific shift. For example, a wanderguard task is documented on the 15th of every month, on the Day shift.

Monthly Duration

The task must be documented monthly, during a specific time range. For example, a monthly weight task is documented during the first 5 days of the month.

Monthly Duration tasks can be documented 1 time.

Note

A Monthly Duration task is yellow for every day and every shift for the time range. When the task is documented, it turns green for every day and shift. It is important to ONLY document the task when you actually complete it. If you document Resident Not Available, Resident Refused, or Not Applicable, other staff are not prompted to document the task because it is green.
