MDS 2.0 Kardex

You can add resident-specific details to the resident's Kardex in addition to the pre-populated MDS assessment information by using MDS 2.0 Kardex. The MDS assessment information is based on the last completed Admission, Quarterly, Annual, or Significant Change in Status assessment type. You can edit and insert information about a specific resident, for example, a personalized bathing schedule.

The information you enter into the MDS 2.0 Kardex option appears in the Kardex Report. You can access this report by clicking the krdx link next to the MDS assessment, or accessing the Reports portal. The Kardex Report is a one-page document that can be printed and used by care staff to provide resident care based on recent MDS assessment information.

  1. From the resident's chart, click the MDS tab.

  2. Click MDS 2.0 Kardex.

  3. In the MDS Kardex Responses window that appears, add/edit information in the available fields.

  4. Click the krdx link next to any locked MDS assessment to view the Kardex Report for that resident with the added customized information.

  • The krdx link opens the Kardex Report Setup screen with resident's Name and Number information and the As Of Date pre-populated

  • You must have appropriate security permissions to access MDS 2.0 Kardex.
