MDS 2.0 - Section V - Narrative RAPs

If your facility is using the narrative format for working Resident Assessment Protocols (RAPs), you can complete RAPs triggered by the RAI-MDS 2.0 assessment using a narrative-structured format.

  1. In the MDS tab of the resident chart, click Edit next to the MDS assessment.

  2. Edit Section V.

  3. Click the RAP to edit. The RAP window appears and shows the following information for each RAP triggered:

    • Resident Name, Description of the RAI-MDS 2.0 assessment type, and ARD.

    • Name of RAP Problem Area, a check mark appears if this RAP was triggered, and the RAP Type.

    • Triggered By indicates which response(s) on the MDS assessment triggered this RAP. These triggers follow the matrix for determining which Raps must be worked as per the CIHI guidelines.

    • Location of RAP Documentation field is present so that you can enter the location of where the RAP documentation was entered. For example, progress note, UDA, notes field in the RAP window that appears.

    • You can also select Proceed or Do Not Proceed to address this RAP in the resident's care plan.

    • Enter notes in the Additional Notes section.

  • After a RAP is complete, the Addressed in CP, and Location and Date of CAP Assessment Documentation columns appear the documentation details.

  • You can record your care plan decisions within each RAP or by clicking Care Plan Decisions. Select Yes or No for each RAP triggered, then Save to record your care plan decisions.

  • If Use Existing RAP Standard Statement’is set to Yes in MDS Configuration, you see a Use Standard Statement link to the right of the Additional Notes field within the RAP. This link only appears if the RAP triggered is an existing RAP for a Quarterly assessment type.

  • In the MDS Portal (Clinical > MDS) > In Progress tab, you can see all in progress MDS assessments. Here, you can edit the MDS to work your RAPS.

  • The Narrative RAPs option is enabled with the appropriate security permissions through Clinical > Setup > MDS Configuration > Use RAPS Module: options are Narrative and New RAPS Worksheet.

  • You must have appropriate security permissions for section V of the MDS to work Raps.

  • You must have appropriate security permissions to see the MDS Portal.
