Adding Scoring to Assessments 

After an assessment is created, you can add scoring. You can then add weights, create assessment scoring categories, and set up assessment schedules and triggers based on a score or score range.

Scoring can be added to an assessment in the Ready for Scoring/SPN Setup or On Line sections of the Custom Assessment Library. 

Some of the System assessments have scoring already assigned, e.g. Braden.

If you create a score based on risk, you need evidence-based documentation to back up the score. 

  1. Click scoring for the assessment.

  2. Description - enter a name for the score.

  3. Default Score - select this if there is only one score for the assessment, or if there are multiple scores and this score is the default/primary.

  4. Max Score - select this and enter a value if you want a maximum score for the assessment. For instance you might create assessment scoring categories that deem a resident to be high risk at a score of 10, so even if they score 15 in the assessment, a score of 10 appears. Leave cleared if you prefer to see the entire score for the assessment.

  5. Exclude from Summary - if selected, the score does not appear in the Assmnts tab, however it is still part of the overall scoring of the assessment and is included in reports. You can use Exclude from Summary if an assessment has multiple scores, or scores that roll up into larger scores.

  6. Formula - Build the score by selecting any of the below options, the mathematical symbol, and clicking Insert. Follow the order of operations for mathematical calculations: Brackets, Exponents, Division, Multiplication, Addition and Subtraction (BEDMAS).

    For example: select CustA1 > click Insert > click + sign > select CustB1 > click Insert > click + sign > select CustC1 > click Insert.

    • Question Key - select an assessment question to pull into the formula.

    • Score - select a score to pull into the formula, if applicable. If no scores are available, this field is blank.

    • Constant - select a constant to pull into the formula, if applicable. For instance if you want to divide a total by two, enter a constant of 2.

      For example, if you want to add two questions together and divide the total by two: click ( sign > select CustA1 > click Insert > click + sign > select CustA2 > Insert > click ) sign > click / sign > enter Constant of 2 > click Insert.

  7. Weights - Click weights for the score and review the Score Value column. If you used numerical values for responses when creating the assessment questions, and the values are weighted correctly, no action is required. If you used letter values for responses when creating the assessment questions, you must change the letter to a number in the Score Value column.

  • The following options are available for scores, however they can also be set up based on a score range, as opposed to a specific score.

    Adding Assessment Scoring Categories

    • Schedule - click schedule for the score to set up an assessment schedule based on an assessment score. Click New>enter Description, select Triggering Type of Score, enter Score Value, select Trigger Assessment, select schedule>click Save.

    • Trigger - click trigger for the score to set up a trigger based on an assessment score. Enter a score value>click New for Care Plan Triggers or High Risk Alert Triggers>select item(s)>Save.

  • If you are working with Service Points Billing and you build several scores, and need to total those scores (eg. Total Care Level): select Score, Insert, +, select Score, Insert, +, and so on.
