Resident Demographics

Most of the resident's demographic information is entered at the time of admission. After a resident is added to the system, you can add or modify resident demographic information. Depending on security permissions, you can view or edit this demographic Information in 1 of 2 ways.

  • From the Resident list screen, you see a View or Edit link to the left of the resident name.

  • From the Resident Header > Edit area, you see an option for Demographics in the edit list. 

 Save is only available if you can edit the demographics for a resident.

  1. From the resident header > Edit menu, select Demographics. You can also select the edit link next to the resident name from the resident list screen.

  2. Enter all available resident specific information into the fields.

    Note

    If a Health Information Exchange (HIE) is enabled in the facility, a Grant Consent to Health Information Exchange section appears and must be completed. This provides consent for information to be passed to an HIE or RHIO. This can include ADT messages on Admission, Discharge, and Transfer, and CCD information on Death, Discharge, and Transfer. If it is not completed, the HIE either does not receive or store the information in their system. It should be completed on patient admission, or if the patient wants to change status.

  • It is recommended that you complete as many of the information fields as possible because these fields populate reports and assist you with providing services and maintaining the resident's account.

  • Allergies - If, at the time of entering the resident information, the allergies are not known, retain To Be Determined. If you know the resident has no known allergies, you can select No Known Allergies. If allergies are known, enter them into the resident Clinical Chart using the Allergies tab.

  • If the resident is discharged and there is a balance to be brought forward for collection, minimal information is needed. Complete the resident's name, date of birth, and previous address information.

  • You must have appropriate security permissions to edit the Demographics section for a resident.

  • The fields that appear on this page are determined by Administrative set-up.

  • Blue field labels indicate information is auto-populated directly in the MDS assessment.

  • The Initial Admission Date and the Most Recent Admission (re-entry) should be the same unless the resident is discharged from the facility and returned or transferred out overnight or admitted to hospital and since returned. In most cases, you cannot enter these dates because they will be grayed out. These dates are populated through the Quick ADT or by a census entry.

  • Disposition, Physician, and Order Review information is typically completed by clinicians.

  • Select Waiting list if this record is that of someone waiting to be admitted or waiting to be admitted to another bed type.
