Profile Module Overview

The Profile module for a resident lists all of the resident's Contact information, Medical Professionals, Custom Information as appropriate to the resident and designated External Facilities. Information contained in the Profile is used in part to build resident admission record, as well as populate discharge and transfer forms.

  • Clinical menu > Residents > Select Resident name link > Profile tab.

  • The Contacts section allows you to record contact information for the resident. Billing, power of attorney (POA), and substitute decision maker (SDM) phone numbers and addresses can be recorded in this area. From the Contacts pane, the Admission record and Transfer/Discharge Record can be accessed via quick links.

  • The Medical Professionals section provides the phone number and address of the medical or healthcare professionals assigned to the resident's care.

  • The Custom Information section is customizable by the facility and is used to record information that is not captured anywhere else in PointClickCare. For example, whether the resident owns his/her own wheelchair or the phone number of the resident's room.

  • The External Facilities section tracks the preferred external facilities for the resident. Most facilities use this section to track items such as pharmacies, hospitals, churches, and labs.
