MDS 2.0 - Submissions - Create a Contact Information File

Based on the Canadian Institute for Health Information (CIHI) rules, a Contact Information file needs to be submitted before your first submission from PointClickCare, and any time there is an update/change to your contact information. CIHI also requests that the Contact Information file be submitted annually before the First Quarter (April 1 to June 30) submissions, regardless if there are any changes or not. If you are required to submit contact information to your Health Authority/Provincial submitting body, follow their guidelines for submission.

  1. Hover over the Clinical tab.

  2. Under the Care Management section, select MDS to access the MDS Portal. Click the Batches tab.

  3. In the Other Submissions to CIHI section, click Contact Information.

  4. A CCRS Contact Information Submission window appears for you to adjust the Fiscal quarter for this submission, Fiscal year for this submission, CCRS Facility Identifier, and it shows the Date Contact information was last exported. Adjust the fields as needed and select Next.

  5. The Export Results window shows the Description, Created On info, Created By info, and the File Name. Select Save As.

  6. Save the file where you can find it. You may want to create a folder specifically for the MDS files. After you select where you want to save the file, click Save.

  7. Open the File. If the file ends in .zip, then it is ready for submission. If the file ends in .txt, you need to convert the file to .zip. Right-click the File > Send To > Compressed Zipped Folder. This creates another file with the same name, ending in .zip. If this is your first time compressing (zipping) files, be sure to verify the process with IT.

  8. Log into the CIHI site with your credentials. Retrieve the .zip file from where you it saved on your computer, and Submit. If you are required to submit contact information to any other submitting body than CIHI, follow their guidelines for submission.

  • If you saved the Contact Information file, but you cannot locate the file, repeat the steps in the procedure to generate it again.

  • The file naming convention as per CIHI requirements (file name is automatically generated by the software) is CCRyyyypphhhhhsss.txt or .zip:

    • CCR - CIHI System.

    • yyyy - Year.

    • pp - Quarter.

    • hhhhh - Facility Number.

    • sss - Sequence Number.

  • Your facility's contact information pulls from Setup, and you need the appropriate security to make any changes or update this information. Clinical > Setup > Minimum Data Set (MDS) - Version 2.0: 'Facility Submission Parameters' contains the CCRS Facility Identifier/Facility number and 'Contact Information' contains all the information about the key people in your facility related to MDS submissions.

  • You must have appropriate security permissions to generate the Contact Information file from the Batches screen.

  • If you submit to CIHI you need credentials to access their website for submission, these credentials come from CIHI.
