Census for Clinical Only – New Census Entry

After a resident is admitted in the system, census entries can be completed through the Quick ADT application while in the resident chart, or through the Admin tab in the Census/Rates module. You can conduct different types of census entries.

  1. Admin > Resident chart > Census/Rates tab.

  2. Click New to open the New Census Entry window.

  3. Enter information for new census entry.

    • The Effective Date.

    • The Effective Time.

    • The Action should reflect the census change that is occurring.

    • Ensure the Payer type is accurate and for Payer Option use the last known to the resident.

      • If the Payer is Medicare, select the appropriate Payer and answer whether or not this is the Start of Medicare Stay? If yes, fill in Days used previously number field if known.

    • Payer Options allows you to select whether to Create New Payer Setup file or to Use Last Known Payer Setup file for Clinical only facilities, select Use Last Known Payer Setup file. In Clinical Only facilities, select Use Last Known Payer Setup file unless the following circumstances apply:

      • If resident is Medicare A and transfers out during the Medicare Stay. On return select the Create New Payer Set up file option to correctly split the MDS Calendar. Unless resident was out for less than 24 hour observation, then select the Use Last Known Payer Setup file option as you don't want MDS Calendar split.

      • When resident changes from Medicare A to another payer, use the Create New Payer Setup file is selected by default. Leave this selected to cancel remaining PPS Schedules in the MDS Scheduler.

    • The Status field shows options that are appropriate based on the Action completed above. Ensure the correct status option is selected.

    • You can leave the Care Level field blank for Clinical Only facilities.

    • You can leave the Alt Care Level field blank for Clinical Only facilities.

    • The Reset Initial Admission Date check box defaults to the correct option, unless a resident is 'Discharged with Return Anticipated' and then subsequently out of the facility for greater than 30 days. See the information link for further information.

  4. In the Location field, assign a room for the resident. Click the lookup.gif icon for a list of available rooms. If the room location selected is incorrect, click the clear link then search.

  5. The Room Rate list is automatically completed when you select the room. You can change this field by selecting an option in the list.

  6. If the census entry is recording the transfer of a resident to/from a different facility, select the type of facility the resident is being transferred to/from, in the To/From Type list.

  7. Enter the Qualifying Hospital Stay Dates if applicable for the payer selected.

  8. Enter in the Type of Admission and the Source of Admission as required.

  9. If the census records the transfer of a resident to or from a different facility, select the name of the facility in the To/From Location list. This is a required field though not needed in a Clinical Only facility.

  • Different types of census entries that you can conduct:

    • Re-admitting a resident.

    • Recording an internal transfer where a resident is moved to another bed and a bed swap where two residents are switched in a facility.

    • Discharging a resident.

    • Recording a transfer in on return from hospital.

    • Recording a leave of absence and return from leave of absence.

    • Recording a transfer out to hospital.

    • Recording the death of a resident.

    • Recording a Payer change.
