MDS 2.0 - Submissions - Correcting Accepted Assessments

You can create a Significant Correction of an MDS assessment if you need to amend some information in an MDS that is already accepted by CIHI. You can also use Significant Corrections to address "Accepted With Flag" records noted in Submission Reports from CIHI. Only assessments that are ACCEPTED by CIHI can be corrected using this process.

  1. Go to the resident chart.

  2. Click the MDS tab.

  3. From the MDS assessment list, click the correct Link to the left of the assessment you want to correct in CIHI’s system. The Modify/Inactivate MDS window appears. The terminology used in PointClickCare is "Modification." This allows you to request a correction of an already accepted assessment.

  4. To correct a record submitted and accepted by CIHI, select Create Modification Request. This option creates a significant correction of the assessment. The status of the original assessment is updated from Accepted to Modified. The newly-created Significant Correction assessment appears above the Modified assessment with a status of "In Progress".

  5. Click the Edit link to the left of the in-progress Significant Correction to make the necessary corrections.

  6. Complete and lock the MDS.

  7. Add it to a Batch.

  8. Export Batch, and submit to CIHI.

  9. After the Detailed Submission Report is available in CIHI, review and mark the MDS as Accepted or Rejected in PointClickCare based on that report.

  • If an assessment is rejected with errors from CIHI during initial submission, this means the assessment does not exist in CCRS. You cannot request a Correction (or Deletion) of that record. A rejected assessment must be marked as Rejected in the batch, unlocked. The necessary changes need to be made, locked, and then submitted to CIHI in a new batch.

  • The correct link is not available for an MDS assessment if the MDS is not yet marked as Accepted within the batch.

  • The following data elements cannot be corrected. If an error is made involving one of these data elements, the record must be deleted (see Deleting Accepted Assessments) from CCRS and a new record submitted:

    • Health Record Number (A6a).

    • Unique Registration Identifier (AA1).

    • Admission/Re-entry Date (AB1).

    • Admission Type (AA7) – on Admission/Re-entry record.

    • Assessment Reference Date (A3).

  • Two data elements are used by CIHI identify the specific record that requires a correction. These are: Unique Record ID (Y1) and Record Type (Y2). For a correction to be processed, these two data elements must match an existing record in CCRS. You can see this information by running a Verification Audit Report (VAR) from CIHI.

  • The Detailed Submission Report and Verification Audit Report (VAR) are CIHI reports and if you have a question/concern about either report, contact CIHI.

  • You must have appropriate security permissions to correct an accepted MDS assessment.
