Care Plans help determine the day-to-day requirements of residents and focus on their special needs. You can create a Care Plan using the focuses, goals, and interventions in the provided library, by creating custom items, or by adding triggers that flow to the Care Plan from Assessments. The items can be further tailored to the resident when an item is added to their Care Plan. 

Care Plans should be reviewed for accuracy quarterly and with any significant change in resident condition.
