MDS 2.0 - Submissions - Correct an Assessment with Effective Date Prior to April 1, 2012

A Significant Correction of an MDS assessment can be created if you need to amend some information within an MDS that has already been accepted by CIHI. To submit a Significant Correction for any assessment accepted by CIHI with an ARD or effective date before April 1, 2012, the assigned Y1 value needs to be added. The Y1 value is assigned by CIHI to the record and needs to be verified by running a Verification Audit Report (VAR) from CIHI.

  1. Request a Verification Audit Report (VAR) from CIHI and locate the assigned Y1 value for the assessment (Y1 is referred to on the VAR as the 'Unique Record ID' – this usually begins with mCIHIcihi...).

  2. In the MDS tab of the resident chart, click the correct link to the left of the MDS. In the Modify/Inactivate MDS pop-up you see a message in red.

  3. Enter the Unique Record ID from the VAR in the 'Y1 =' field. Then click Create Modification Request to create the Significant Correction. The status of the original assessment is updated from 'Accepted' to 'Modified', and the newly created Significant Correction assessment appears above the Modified assessment with a status of 'In Progress'.

  4. Click the edit link to the left of the in progress Significant Correction to make the necessary corrections.

  5. Complete and lock the MDS.

  6. Add it to a batch.

  7. Export batch and submit to CIHI.

  8. After the Detailed Submission Report is available in CIHI, review and mark the MDS as Accepted or Rejected in PointClickCare based on that report.

  • When batching the Significant Correction, remember to adjust the Fiscal Quarter and Fiscal Year to find it.

  • The Verification Audit Report (VAR) is a CIHI report and if you have a question/concern about that report, contact CIHI.

  • You must have appropriate security permissions to correct an accepted MDS assessment.
