Creating New MDS 2.0 Assessment

You can create a new MDS Assessment for a resident. The MDS Assessment can be saved, signed, locked, and completed to allow for submission as applicable.

A new MDS Assessment can be created in multiple ways:

  • The New button in the MDS tab of the resident clinical chart.

  • The schedule links next to the Next Full and Next Qtrly fields in the MDS tab.

  • The copy links next to an MDS assessment already contained in the MDS tab.

  • The MDS portal views, using the schedule or copy links.

You can create the following MDS records depending on the type of record you need to create:

  • 01. Admission assessment - options to create one or both of an Admission Face Sheet or Full Admission Assessment.

  • 02. Annual assessment.

  • 03. Significant change in status assessment.

  • 05. Quarterly review assessment.

  • 06. Discharge (return not anticipated).

  • 07. Discharge (return anticipated).

  • 08. Discharged before completing initial assessment.

  • 09. Re-Entry.

  • Update record.

  • Transfer record.

  • Special project record.

If RUG Optimizer is enabled, you can click the link in the Reason for Assessment window to view the RUG Optimizer for Admission, Quarterly, Annual and Significant Change in status assessment types.

  1. From the Resident Clinical chart, click the MDS tab.

  2. In the Reason for Assessment popup, select and enter the appropriate information for your Assessment type.

  • Depending on the Reason for Assessment selected and the Province of the facility, various fields and options appear.

  • If Reason for Assessment = Admission assessment (required by day 14), then select whether a Face Sheet, Full Admission Assessment or Both Face Sheet and Full Admissions Assessment are created.

  • Alternatively, you can open a New MDS Assessment from any scheduled MDS link or the copy links found in the MDS tab and the MDS portal.

  • The ARD selected is based on facility MDS schedule and can be future or back-dated depending on your schedule.

  • If the Reason for Assessment = Admission assessment and either a Face Sheet or Both is selected, you are prompted to select whether or not to generate a New URI (and New ACCIS Resident Identifier for Alberta facilities).

  • If an Admission assessment or Re-Entry assessment is selected, you may be prompted to select whether or not to generate a New Register Number. This option is based on configuration.
