Understanding Readmissions Analysis Calculations

The measures that are available in the Readmissions Analysis module are built off of the Resident Stay Record. The following information outlines how the measures are calculated as well as common terms or concepts that are used in both the calculations as well as the dashboards themselves. Common terms used in the definition of the calculations as well as seen on the dashboards as selection options within the Readmission Analysis module are as follows:

  • Metric Days - a pre-defined selection option. The number of days from the date of admission that is being evaluated within the rate.

    • Metric Days distinguish between the selection of a 30-day readmission rate versus a 60-day readmission rate.

  • Period - the time frequency for which the measure is calculated.

    • If the month and year selected includes the most current data available, then the most current periods are a partial representation because not enough time has elapsed for the run-off window to be complete.

  • Month and Year - defines the span of time represented in the display of the dashboard.

Readmission Rates - The readmission rates within the Readmissions Analysis module are calculated off of the Resident Stay record. The calculation is the observed rates for those residents that are admitted into the facility from an acute care hospital and are discharged back to an acute care hospital.

Available rates include Overall Readmission Rate.

  • Numerator - of those admissions defined in the denominator, the number of resident admissions which were discharged back to an acute care facility within X days (metric days) of the admission date.

  • Denominator - the number of resident admissions to the facility from an acute care facility within the selected period, month and year.

  • The Overall Readmission Rate is the default rate within the module and is calculated as described above with no additional criteria.

Discharge Rates - The discharge rates within the Readmissions Analysis module are calculated off of the Resident Stay record. The calculation is the observed rates for those residents that are admitted into the facility and are discharged from the facility. Available rates include Overall Discharge Rate and Discharge Rate to Acute Care Facility

  • Numerator - of those admissions defined in the denominator, the number of resident admissions which were discharged within X days (metric days) of the admission date.

  • Denominator - the number of resident admissions to the facility within the selected period, month and year.

  • The Overall Discharge Rate is the default rate within the module and is calculated as described above with no additional criteria.

  • The Discharge Rate to Acute Care Facility is calculated as described above however the numerator definition is augmented to consider only those resident admissions that were discharged to an acute care facility within X days (metric days) of the admission date

Risk Factor Rates - The risk factor rates within the Readmissions Analysis module are calculated from the Resident Stay record. The calculation is an observed rate for those residents that are identified with specific criteria that flags them as a possible resident of interest. The risk factor rates are not predicting that a resident can be readmitted, but rather they are a means to categorize the residents. Available rates include Overall Risk Factor Rate.

  • Numerator - of those admissions defined in the denominator, the number of resident admissions meeting one of the defined risk factor criteria

  • Denominator - the number of resident admissions to the facility within the selected period, month and year.

  • The Overall Resident Risk Factor Rate is the default rate within the module and is calculated as described above however the numerator definition is augmented to consider the risk factor criteria of only those resident admissions that were admitted from an acute care facility.

Average Length of Stay - The length of stay metrics within the Readmissions Analysis module are calculated off of the Resident Stay record.  Length of Stay measures the length in days between the date of admission and either the date of discharge or the current date should the resident be in an active stay. Average Length of Stay is only calculated for those residents that were admitted from and discharged to an acute care facility. Average LOS = date of discharge (or today if no discharge has occurred) - date of admission.

  • Numerator - of those resident admissions defined in the denominator is the total (sum) of length of stay in days for those resident admissions.

  • Denominator - total number of resident admissions from an acute care facility that resulted in a discharge to an acute care facility within the selected period, month and year.

  • The Average Length of Stay is the default rate within the module and is calculated as described above with no additional criteria.

  • The Length of Stay represents the number of days between admission and discharge within the module.
