Viewing, Unlocking, or Editing a Historical Incident

After an incident report is locked, it is stored in the Historical section of the Risk Management Portal. You can view the completed records from this section and can also unlock a record if editing is required.

To view an Historical record

  1. Clinical > Risk Management > Historical

  2. Click the view link next to the desired Incident Record. The Incident Record appears.

  3. Use the Incident Record tabs; Details, Injuries, Factors, Witnesses, Action, Notes, and Signatures to navigate to the appropriate section that you want to view.

To unlock and/or edit an Historical record

  1. Clinical > Risk Management > Historical

  2. Set filter View for Last to All, or 1 to 3 months as needed.

  3. Click the unlock link next to the Incident Record that requires reopening. The Incident Record appears. This record appears in the Active view and you can change it.

  4. Edit as needed then sign and lock.

  • You must have Unlock Incident Record security permission to unlock an historical record.
