Calculating Hospital Rates Quick Reference Guide

This screen contains items required to calculate Hospital Rates in the Hospital Tracking Portal. 

Note

For the Hospital Rates to calculate correctly, all fields for the census event must be completed.

1.

Resident Days

Each day a resident spends in a facility contributes to 1 resident day for the month. 

Note

Resident Days in the Hospital Rates tab are not the same as Census Days. Resident days are always greater than Census Days because Resident Days include the day a resident is discharged from a facility, whereas Census Days do not.

Residents Admitted to Hospital

The number of transfers to a hospital with an outcome of Admitted, Inpatient; Admitted, Status Uncertain; or Admitted, Observation during the selected month.

30-Day Readmission Rate

The 30-Day Readmission Rate calculates data from the Admission Log and Transfer Log for the selected month.  

Note

For the 30-Day Readmission Rate, a full month is required for the calculation. N/A appears before the full month is complete.

Using the example of May 2018:

Use the Admission Log to determine the denominator for the calculation

  1. From the Admission Log, click the plus icon next to Display Filters to see filter options.

  2. Type or select the month, then select Yes for Include Rate.

  3. Click Search.

  4. Record the number of Post Acute admissions, Chronic Long-Term admissions, and the total number of admissions.

    5 Post Acute + 2 Chronic Long-Term = 7 total admissions.

Use the Transfer Log to determine the numerator for the calculation

  1. From the Transfer Log, click the plus icon next to Display Filters to see filter options.

  2. Select the filter options.

    • Type or select the month.

    • Select View Transfers by Hospital Discharge/Admission Date Range.

    • For Planned, select No.

    • For Outcome, select Admitted, Inpatient; Admitted, Observation; and Admitted, Status Uncertain.

    • For Length of Stay, select <30 days.

  3. Click Search.

  4. Record the number of Post Acute transfers, Chronic Long-Term transfers, and total number of transfers.

    0 Post Acute + 1 Chronic Long-Term = 1 total transfer.

    Note

    If the Purpose of Stay on the Transfer Log does not match the Admission Status on the Admission Log for a resident, use the Admission Status (Post Acute or Chronic Long-Term) for the transfer.

Complete the 30-Day Readmission calculation using the above numbers.

  1. The calculation for Post Acute is 0 transfers divided by 5 admissions = 0 x 100 = 0.0%.

  2. The calculation for Chronic Long-Term is 1 transfer divided by 2 admissions = 0.5 x 100 = 50.0%.

  3. The calculation for All Residents is 1 transfer divided by 7 admissions = 0.143 x 100 = 14.3%.

Hospital Admission Rate

The number of hospitalizations divided by Resident Days multiplied by 1000. 

Using the example of May 2018:

  • The calculation for Post Acute is 2 divided by 557 = 0.00359 x 1000 = 3.6.

  • The calculation for Chronic Long-Term is 3 divided by 384 = 0.00781 x 1000 = 7.8.

  • The calculation for All Residents is 5 divided by 941 = 0.00531 x 1000 = 5.3.

Transfers Resulting in ED Visit Only

The number of ED Visits divided by Resident Days multiplied by 1000.

In the example for May 2018, there are no ED Visits for the month.

Using the example of June 2018, there is 1 Post Acute transfer with an outcome of ED Visit Only. 

  • The calculation for Post Acute is 1 divided by 576 = 0.00173 x 1000 = 1.7.

  • The calculation for Chronic Long-Term is 0 divided by 403 = 0 x 1000 = 0.0.

  • The calculation for All Residents is 1 divided by 979 = 0.00102 x 1000 = 1.0.

Transfers Resulting in Observation Stay

The number of transfers resulting in an observation stay divided by Resident Days multiplied by 1000.

Using the example of May 2018, there is 1 Post Acute transfer with an outcome of Admitted, Observation.

  • The calculation for Post Acute is 1 divided by 557 = 0.00179 x 1000 = 1.8.

  • The calculation for Chronic Long-Term is 0 divided by 384 = 0 x 1000 = 0.0.

  • The calculation for All Residents is 1 divided by 941 = 0.00106 x 1000 = 1.1.
