Capturing Wound Images in the Skin & Wound Mobile App

Using the PointClickCare Skin & Wound mobile app, you can capture images of a wound to track measurements and wound healing progress. Accurate and consistent measurements are critical for determining if a wound is improving or deteriorating, and allows for early and appropriate intervention. 

A HealX is a sticker specifically designed for precise measurement and lighting correction, and must be used when capturing the wound image. The HealX must be placed next to and on the same plane as the wound each time the wound image is taken. After documentation is complete, remove and dispose of the HealX.  

Warning

In areas where internet connectivity is low, you are warned that information has not yet synced. To prevent data loss, remain logged in and ensure connectivity for syncing to occur. If you do not sync before logging out, your data is lost.

  1. Remove the dressing and cleanse the wound per protocol.

  2. Place a HealX next to and on the same plane as the wound on the resident's intact skin.

  3. Position the camera parallel to the wound, approximately 12 inches away, so that the wound is centered in the screen.

    The image must be clear and show the wound and peri-wound area within the image screen. 

    • Use the camera's zoom option if needed to size the wound on the screen.

    • When capturing an image of a large wound, you can move the camera greater than 12 inches away, so that the wound is centered in the screen. 

    • To focus the camera on a wound, simply tap on what you want to be the focus. When you tap, you see a yellow box flash where you want the camera to focus. Let the camera quickly adjust to where you want to focus.

    • The HealX turns orange when capturing an image to assist in positioning the camera for photo and measuring consistency.

  4. Click the shutter button. If image is poor, a message appears allowing you to retake the photo.

    Note

    To improve the coloring of the captured image, you can adjust the color balance by tapping on the color balance icon. After you save the color adjustment, the shaded circle shows a green dot. To remove the color adjustment, retake the photo.

  • If the wound is in an awkward spot, get someone to help stabilize the area.

  • To capture an image for a wound on a toe, you can place the HealX on the foot, on an adjacent toe, or on a card next to the toe.

  • To capture an image for a heel wound, position the resident to expose the heel, then position the camera to focus on the largest area of the wound.

  • Wound images are not stored on the mobile device, and are only stored in assessments in the mobile and web applications.

  • The Skin & Wound module has a Disable Visual History option. Disable Visual History allows you to use the mobile app without saving wound images to wound assessments. When saving a wound assessment, only the trace overlay with measurements is saved. Disable Visual History must be enabled for your organization or facility through PointClickCare
