Incident Analysis Report

The Risk Management Portal is separate from the resident record and, as such, reporting is only accessed from the portal.

  1. Click the Incident Analysis link under the Reports pane. An Incident Report Setup window appears. Select various reporting options around incidents documented in the Risk Management Portal.

  2. Select a single resident using the magnifying glass lookup.gif to the right of the Resident Number or Resident Name. If a report for more than one resident is desired, leave these fields blank.

  3. Resident Status  select whether to show Current, Discharged or Both sets of residents and the Unit and Floor lists are available to select resident incidents by Unit or Floor.

  4. Select a specific date range by using the Calendarcalendar.gif icons or you can select to view one month of incidents by clicking the Use One Only link under the date range option.

  5. Use Include Incident Types to select the specific types of incidents to be viewed on the report. Viewing a Struck Out Incident Report provides strike out information in the header.

  6. Use Include Incident Locations to select to see specific locations of incidents occurring on the report.

  7. Use Include Only Incidentswhere the following Agencies/ People were Notified to further select incidents where a specific Agency/ Person was notified.

  8. Use Include Only Incidentswhere the following Agencies/ People were NOT Notified to further select incidents where a specific Agency/ Person was NOT notified.

  9. Use Predisposing Factors to select specific predisposing factors on the report.

  10. The next options allow you to select one of Include Only Incidents with Injuries, Include Only Incidents without Injuries or Both: Include Incidents with and without Injuries. You can further filter the type of injury sustained via the options.

  11. Use Include only Incidents where Resident was sent to Hospital to report on all resident incidents where Resident Taken to Hospital? under the Details tab of the incident is selected.

  12. Further options to show additional information is available: Injury, Location, Witnessed, Sent to Hospital and Admission Date. 

  13. You can sort the report by either Incident Type, Injury Type, Location of Incident, Resident or Incident Date and Time.

  14. Click Print at the bottom of the Incident Report Setup screen to view the report.
